It's always off the highest fare. I cant confirm as for international but yes, everyone pays taxes even if you work for the airline. Check your interline agreements and see which airlines charge flat fee for miles plus tax because those are usually cheaper and will save you at least 40-50% of the ID fares.
Most of the times ID90 is rhe lowest Unrestricted fare but it have to be Y.
Ex DL has a Y0BV fare which qualified and was better than the regular Y fare.
Attention there is often a minimum fare on a lot of ID90 which is 25USD plus taxes

We are allowed 9 round trips per anniversary year where all we pay are the taxes..after the 9 round trips we have to pay yeild fare just as our travel companions/buddy pass riders pay...
Taxes paid by employee or spouse depend on the country.
UK departure runs about $50 (if I remember correctly) while Chile and Japan had no taxes. Those are the only countries I have traveled to so they are the only ones I am sure of.
