If the counter agent fails to identify to large of a carry-on(or if the pax check in online/kiosk) then it is the gate agents responsibility. If the gate agent fails to do his/her job, then it falls on the FA. If the bag has to be checked last minute, it will be loaded in the bin and counted as a standard checked bag on the cargo load report. If the bag can be stuffed into the cabin somewhere, then it is counted as part of the weight of the passenger, and thus, not reported on the cargo load report.
